INITIAL APPLICATION

Any local disc organization that is interested in becoming an affiliate of USA Ultimate should first submit an application.

Application to Become an Affiliate of USA Ultimate  (Online form)

Once an organization's application has been approved, the organization can then submit the following compliance documents:

a.

Affiliate Operations Manual and Agreement (PDF File)

Open file in Adobe Acrobat in order to type in details electronically. Fill out Appendix A and have the organization president sign at bottom of page 10, retaining a copy for the organization's records.  Electronic signatures will be accepted.

b.

Annual Board Report & Feedback (Online survey)

Help USAU staff effectively track, report, serve, and support your authorized contacts, positions, goals, and strategies.

c.

Proof of Directors and Officers insurance (D&O) coverage or a completed D&O application.  

Coverage is available through USAU's carrier for as low as $550/year, or have USAU cover the premium (up to $750/year) with an initial 3-year (vs 1-year) commitment to the Affiliate Program.

d.

Application for Local Outreach Representative (LOR)

Let us know who the designated contact for your organization will be to respond to any public inquiries about local ultimate opportunities and programs. We want to help connect new players to your community and get them steered in the right direction to join you!

      

ANNUAL COMPLIANCE

Existing affiliates will be asked to ensure completion of the following things each year, in order to qualify for the annual membership rebate payment from USA Ultimate:

1.

Sanction each of the Affiliate's owned and operated events and programs for the local ultimate community served by the organization, with full sanctioning program compliance (signed, current memberships, safety guidelines, etc), with a minimum of 2 total events/programs (leagues, tournaments, etc) including at least one youth event/program.

2.

Send at least one representative to the annual Organizers Convention that has recently become a standalone, winter off-season event, hosted in conjunction with an annual coaching conference.

3.

Run a minimum of one USA Ultimate Outreach or Education program (e.g., GUM, LTP, CDP, OCP, PE outreach, etc) in the local community each calendar year.

4.

Renew the D&O insurance application as needed, based on individual policy dates, in order to maintain appropriate coverage of your board, officers and/or staff decisions and actions.

5.

Complete the annual Affiliate Board Report & Feedback survey including the component that requires a renewal of the Operations Manual and Agreement as well as the component that confirms or updates the designated Local Outreach Representative (LOR)

Report is due within 30 days of distribution of annual reports (sent by HQ staff to Affiliate contacts/leadership) each year; the report especially helps USAU staff to:

i. Effectively track, report, serve and support authorized contacts, goals, and strategies;
ii. Thoroughly evaluate the affiliate program to identify potential improvements;
iii. Clearly affirm the continuing relationship between organizations;
iv. Identify the appropriate person to be listed as the public contact for inquiries about your area.
