How to Order a Gift Certificate
Click the below button to fill out a brief Google form and describe the recipient and membership type that you'd like to gift.
Be ready to enter the following information:
- Purchaser Name and Email Address
- Recipient Name and Email Address
- Membership level to gift (i.e., Youth, College, Adult, Lifetime, etc.)
- Term/duration of membership (1, 3, 5 yr, etc.)
- Personal Message (optional)
*Requires payment by PayPal. An invoice will be sent by a staff person to the email address provided.

Note: To have your gift received before 12/25, please make purchase no later than 12/18.
USA Ultimate Membership - USA Ultimate memberships are not refundable. Members are encouraged to sign up as early as possible to receive the most from their membership. All USA Ultimate memberships are based on the calendar year (January 1 – December 31). Any memberships purchased through November 30 will expire on December 31 of the current year. Memberships for the following year become available on December 1. Please review the information carefully before registering.
